Homoeopathic Approach in the Management of Pilonidal Sinus

Pilonidal sinus disease (PSD) is a chronic disorder of the natal cleft that predominantly affects adolescents and young adults. It may present as an acute abscess or as a chronic discharging sinus, with symptoms including pain, swelling, local tenderness and intermittent discharge. The contemporary understanding of pilonidal disease supports an acquired pathogenesis associated with hair penetration, follicular obstruction and local mechanical factors. Management depends upon the clinical presentation and may range from local care and observation to incision and drainage or definitive surgical procedures. Homoeopathy approaches patients on the basis of individual symptom patterns and traditionally employs materia-medica and repertorial methods for remedy selection. In this article, the application of Boger Boenninghausen's Characteristics and Repertory (BBCR) to symptoms associated with pilonidal sinus is discussed. Selected remedies traditionally considered in suppurative and chronic sinus conditions, including Silicea, Hepar sulphuris calcareum, Myristica sebifera and Calcarea sulphurica, are described from the homoeopathic materia-medica perspective. The article primarily presents a homoeopathic and repertorial perspective and should not be interpreted as evidence that homoeopathic treatment can replace established surgical management of acute abscesses or complicated pilonidal disease. Further well-designed clinical research is required to determine the effectiveness and safety of homoeopathic interventions in this condition.
